These forms require the free Adobe Reader or other PDF viewing software. In order to assist you in your recordkeeping and tax reporting, we want to inform you that the Society may provide you with a Form 1099-MISC or Form 1042-S, as may be applicable. We recommend that you consult with a tax advisor if you have any questions regarding your filing obligations or tax liability.
Form 1: U.S. citizens, U.S. residents and U.S. entities. (Form W-9 SUB)
Form 2: Non-U.S. entities or individuals receiving funds for work conducted solely outside the U.S. (Form W-8 Replacement)
Form 3: Non-U.S. individuals claiming treaty benefits for funds received for work conducted wholly or partly inside the U.S. (Form 8233 and Instructions)
Form 4: Non-U.S. entities or individuals receiving funds for work conducted wholly or partly inside the U.S. who are NOT claiming treaty benefits and for whom payment is not "effectively connected" with a U.S. trade or business. (Form W-8BEN SUB1)
Form 5: Non-U.S. tax-exempt entities receiving funds for work conducted wholly or partly inside the U.S. who are claiming exemption from withholding based on tax-exempt status. (Form W-8EXP SUB)
Form 6: Non-U.S. entities receiving funds for work conducted wholly or partly in the U.S. who are claiming treaty benefits. (Form W-8BEN SUB2)
Form 7: Non-U.S. entities receiving funds for work effectively connected with recipient's conduct of U.S. trade or business. (Form W-8ECI SUB)
